Additional information regarding sample selection. - “Index” case: The French National Health Insurance Fund (CNAM) will generate an email each week for cases identified as positive for SARS-CoV-2 by PCR the previous week in the COVID-19 screening information system database, SI-DEP). The emails will contain a generic hyperlink to the questionnaire hosted on the IPSOS institute platform. The cases will be classed as reporting transmission within the home (arm A3) or not (arm A1 if the source of infection outside the home is known, or otherwise A2).

- “Close contact” controls: Close contact invited by the index case, living in their home, and who is not the person having infected the index case, if transmission within the home is suspected. These close contact controls will have had a negative result from a test performed in the context of contact tracing. The controls will be classed according to whether the index case inviting them has reported transmission within the home (arm B4) or not (arm B3).

- “Close contact” cases: Contacted via the same procedure as the “close contact” controls, but having had a positive result from a test performed in the context of contact tracing. These will be considered as cases reporting probable transmission within the home, and will be classed according to whether the index case inviting them has reported probable transmission within the home (arm B2) or not (arm B1).

- “Distant contact” controls: Subjects invited by Ipsos, selected from institute or partner panels, not infected with COVID-19, paired with index cases by age (in age groups up to and including 68 years), gender, population density (by category) and region of residence. These constitute arm C in the diagram. IPSOS invites distant contact controls via emails generated weekly, a week after recruitment of the index cases, then selected based on the pairing criteria. The email will contain a link to the questionnaire hosted on the IPSOS institute platform.
Database objective
Main objective Identify the sociodemographic characteristics, places frequented, and behaviours associated with a risk of SARS-CoV-2 infection.
Inclusion criteria All subjects:
o Adult subject
o Having agreed to take part in the study
o Index case:
o Cases identified in the COVID-19 screening information
system database, SIDEP
o With a positive test for SARS-CoV-2 by
RT-PCR on a nasopharyngeal or throat swab (or
any other sample for which current technology
indicates active infection if positive) within the past 14
days
o Close contact cases:
o Living in the same household (sharing the same
home) as the case having invited him/her to take part
o With a positive test for SARS-CoV-2 by RT-PCR on a nasopharyngeal or
throat swab (or any other sample for which current technology
indicates active infection if positive)
following contact with the index case
o Close contact controls:
o Living in the same household (sharing the same
home) as the case having invited him/her to take part
o With a negative test for SARS-CoV-2 by RT-PCR on a nasopharyngeal or
throat swab (or any other sample for which current technology
indicates active infection if positive)
following contact with the index case
o Distant contact controls:
o Selected by IPSOS based on criteria including age,
gender, and administrative department of residence (paired
with index cases)
