Details of collected declarative data | The social and medical characteristics were collected bilaterally (mirror questionnaires) from patients by telephone (for the social characteristics) and from physicians by questions asked face-to-face by physician surveyors previously trained in administering the questionnaires. The physicians were interviewed in their offices and responded for each patient with the aid of their medical files. |
