Additional information regarding sample selection. 5,000 "successful" people, by voluntary region, i.e. a total of 20,000, were needed for the survey, thus allowing for a representative level of health regions, plus a national sample of 2,000 people with only mobile phones.

It is in stata in .dta format as well as the documentation: questionnaires and description of the variables.
