End of inclusions: 01/12/2011Plus de 10 ans se sont écoulés depuis le début de l’étude EPIPAGE. Les évolutions qui se sont produites depuis justifient la mise en œuvre d’une nouvelle cohorte.
Les objectifs généraux d'EPIPAGE 2 sont :
1) l’étude devenir des enfants grands prématurés et de leurs familles ;
2) l’évaluation des effets de l’organisation des soins et des pratiques médicales sur la santé et le développement des enfants prématurés ;
3) la recherche des causes et des conséquences de la prématurité ;
4) l’étude des parcours des familles et les processus de décision au moment de la naissance et en soins intensifs ;
5) l’analyse des besoins de prise en charge médicale et éducative.
Une originalité de l’étude est d’avoir permis la mise en place de projets complémentaires et multidisciplinaires pour répondre ou approfondir des questions difficiles à traiter sur l’ensemble des enfants de la cohorte. Ces projets, qui s’appuient sur la structure EPIPAGE 2, nécessitent des investigations supplémentaires et un financement propre. Au total, 8 ont été développés dans le champ de l’imagerie cérébrale, des bio-marqueurs, de la nutrition, des interactions mère-enfant et de l’éthique:
- ETHIQUE (Processus de décision qui aboutissent à la survie ou au décès des enfants nés entre 22-26 SA)
- EPIFLORE (Etude de la mise en place du microbiote intestinal)
- EPIRMEX (Etude des fonctions exécutives et du développement du langage en fonction des anomalies détectées à l’IRM )
- BIOPAG (Identification des biomarqueurs précoces de pathologies à partir d’un prélèvement de sang de cordon (ADN, ARN) chez l’enfant et d’un prélèvement sanguin chez la mère (ADN, ARN))
- OLIMPE (Etude des liens existant entre la qualité des interactions précoces mère-enfant et le devenir neuro-développemental)
- CHORHIST (Recherche des signes de chorioamniotite par l’analyse anatomo-pathologique du placenta)
- EPINUTRI (Etude des associations en apports en acides gras poly-insaturés, les apports en fer et le devenir neurologique des enfants grands prématurés)
- EPPIPAIN 2 (prise en charge des gestes douloureux en réanimation néonatale)
Plateforme RE-CO-NAI
EPIPAGE 2 est l'une des deux cohortes constituantes de la plateforme RE-CO-NAI avec la cohorte ELFE. L'objectif général de cette plateforme est de mettre en place une infrastructure offrant les dispositifs nécessaires à la collecte, au stockage hautement sécurisé et à la diffusion de données sur la grossesse, la naissance et l'enfant. La plateforme de recherche ainsi constituée sur les cohortes d'enfants suivis depuis la naissance permettra d'étudier de manière globale et multidisciplinaire les grands enjeux sur la santé, le développement, et la socialisation des enfant. Elle donnera en outre une visibilité importante dans le monde de la recherche académique (française et internationale), mais aussi auprès des instances , associations, industriels qui s'intéressent à l'enfance. Elle permettra ainsi une valorisation optimisée des données recueillies et facilitera la dissémination des données.More than 10 years are passed since the beginning of the EPIPAGE study. The evolutions occurred since justify the creation of a new cohort.
The main objectives of EPIPAGE 2 are:
1) Study the becoming of extremely premature children and their families
2) The evaluation of care organization effects and medical practice on premature children’s health and development
3) Research of causes and consequences of prematurity
4) Study the path of the different families and the decisions process at the moment of birth and intensive care
5) The needs analysis of medical and educational care
One of the originalities of the study is to have permitted the creation of complementary and multidisciplinary projects, in order to reply and deepen to question difficult to handle on the children of the cohort. These projects, based on the same structure of EPIPAGE 2, needs additional investigations and an own funding.
A total of 8 studies have been developed in the field of brain imaging, bio-markers, nutrition, mother-child interaction and ethics:
- ETHIQUE (decision process leading to survival or death of children born between 22 and 26 weeks of amenorrhea)
- EPIFLORE (study of the introduction of the intestinal microbiota)
- EPIRMEX (Study of the executive functions and of the language development, depending on anomalies detected through NRM)
- BIOPAG (Identification of the early bio-markers of pathologies from an umbilical cord sample (DNA, RNA)
- OLIMPE (Study of the existing links between the quality of early mother-child interactions and the neural and developmental becoming)
- CHORHIST (Research of chorioamniontides through anatomo-pathological analysis of placenta)
- EPINUTRI (Study of the associations of the intake of polyunsaturated fatty acids, iron intake and neurological becoming of extremely premature children)
- EPIPAIN 2 (treatment of painful movements in neonatal intensive care.
RE-CO-NAI platform
EPIPAGE 2 is one of the two cohorts constituting the RE-CO-NAI platform, together with the ELFE cohort.
The general objective of this platform is to create an infrastructure offering the measures required for collecting, highly secured storage and distribution of data concerning the pregnancy, the birth and the child.
The research platform consist of cohorts of infants followed since birth will allow to study, in a global and multidisciplinary way, the stakes on health, development and socialization of children.
It will in addition provide substantial visibility in the world of academic research (French and international), but also with regards to bodies, associations, and industries that have interests in children. As such, it will allow for an optimized valorization of the data collected and will facilitate the dissemination of data.EPIPAGE 2 est une étude de cohorte prospective en population.
- Ont été inclus tous les enfants nés, dans 25 régions de France, extrêmement prématurés (22-26 Semaines d’aménorrhée (SA)) sur 8 mois, grands prématurés (27-31 SA) sur 6 mois et modérément prématurés (32-34 SA) sur 1 mois.
- Un groupe témoin d’enfants nés à terme sera constitué à partir de l’étude Elfe.
Les enfants des familles ayant exprimé un refus ne seront pas inclus. Dans ce cas, les informations issues du certificat de santé du 8ème jour pourront être utilisées afin de caractériser la population concernée.EPIPAGE 2 is a population based perspective cohort. All the children born in 25 regions of France, extremely premature (22-26 weeks of amenorrhea) on 8 months, highly premature (27-31 weeks of amenorrhea) on 6 months and moderately premature (32-34 weeks of amenorrhea) on 1 month. A control group of full-born children will be constituted from Elfe study. Children of families having expressed a refusal will not be included.
